Responsibilities

Liberty Health Care Center is accepting aplications for an Admissions Coordinator. Two positions available.  This marketing position includes duties related to planning, organizing and directing admissions. Schedule flexibility is required.

 

Duties include, but are not limited to:

 

  1. Handles initial inquiries/interview inquirers for potential admission.
  2. Develops and visits referral sources (minimum for two contacts per week for marketing).
  3. Coordinates the handling and follow-up for evening and weekend inquiries.
  4. Completes monthly marketing logs, and inquiry and admission/readmission logs.
  5. Serves as the primary liaison with families, guardians, hospitals, agencies and other facilities for admission purposes.
  6. Completes the in-person assessment of an admission where applicable.
  7. Responsible for obtaining and confirming payor source information prior to admission, when possible.
  8. Submits the pre-certification for managed care cases where applicable.
  9. May follow up on initial physical exam of admission with the designated nursing staff.
  10. Completes and distributes admission paperwork, transfer form and PASSAR to the various departments and providers.

Qualifications

Qualified applicant should have experience in the following areas: Marketing, Medicare, Medicaid, private insurance, managed care, resident admission. Must have a current Licensed Practical Nurse (LPN) license in the State of Ohio.  Qualified applicant must be able to work flexible hours including evenings and weekends as needed for admissions.
