Windsor House at Canfield, our brand new facility opening Fall 2017, is accepting applications for a full time MDS Nurse (RN) position. Major responsibilities include, but are not limited to (full job description will be reviewed prior to/at time of offer):


  1. Based on the MDS3.0, makes a comprehensive assessment of resident’s needs which includes; prior medical functional status; sensory and physical impairments; nutritional status; special treatments and procedures; discharge potential; dental condition; activities potential; rehab potential; cognitive status and drug therapy, assessment must be no later than 14 days after date of admission. All resident assessing will be under the direction of the Director of Nursing or the RN coordinator.
  2. May be required, at the time each resident is admitted, to obtain physician orders for immediate care; ensure physician orders are received and signed as required, and enter in computer.
  3. Discharge summary must be completed and a discharge plan of care developed when it is anticipated that resident is to be discharged. May be delegated to a RN, Head Nurse or Social Services. The discharge plan of care must be signed by an RN and all other interdisciplinary team members.
  4. Provides directions to staff nurses in decision making concerning either nursing practice issues or in interpretation of administrative and nursing policies and procedures. This must be directed by the DON or the RN in charge at the time.
  5. Attends in-service programs; professional seminars and workshops to promote both individual and professional growth.


Must be a Registered Nurse in the State of Ohio with two years experience in long term care. Previous experience in a restorative program preferred.
